After School Club Booking Forms

Please complete the google form to confirm your bookings for after school club.

Bookings MUST be made two weeks in advance so that we are able to adequately staff the club.

Your ParentPay account MUST be in credit for you to book sessions.

If you are paying by Childcare Vouchers please let the office know.

There maybe last minute spaces available but you will need to ring the office to confirm.

Likewise - if you have booked and then no longer require the place please ring the office.

Please note if you are late collecting your child there will be a charge of £15 plus £15 for every 15 minutes you are late collecting. The club closes at 5.30pm which is when these charges will become applicable.

If your child is only booked for session 1 and you do not arrive to collect until after 4.30pm you will be charged for the second session also.

Should you need to contact the After School Club between the hours of 3.25pm - 5.30pm please call St Thomas' Day Nursery on 01538 722208.

Regular After School Club Booking Form Please complete this form if your childcare arrangements are the same every week. Should you need to make any changes/additions please let the office know - if possible two weeks in advance. These bookings will be carried forward until we are advised otherwise.
